Name: CardMan 3021 / 3121
According to my experience and information from the people who program the linux drivers for this device (https://ccid.apdu.fr/) the device with the USB ID 076b:3021 is sold as a cardman 3021 (https://ccid.apdu.fr/ccid/idVendor.html#149 ) _and_ cardman 3121 (https://ccid.apdu.fr/ccid/idVendor.html#145). It is just seems to be a different casing (the 3021 is more compact/slim).
