My Sun USB Mouse (Model: FID-638, PN: 371-0788-01, With scrollwheel) has this ID. This mouse is sold together with the Type 7 keyboards.

The Sun Type 7 Keyboards have 3 USB Ports. The FID 638 Mouse shipped with the keyboard mouse bundle by sun seems to be manufactured by Monterey International Corp. (see 0566:4006)
I think "24.1" LCD Monitor v4" is wrong for same reason as "FID-638 Mouse" because both could be connected thru the hub.
